Pretty sure you can still write the appeal in this thread below with the offcial format. If you want your appeal re-considered I strongly suggest you write a new one and don't copy-paste your old one. Your notes do not inspire much confidence as you have three for attempted RDM, two of them for welderbombing within a week of one another.
